A claim assessment records what was considered, what disagreed, and the confidence the model actually held.
Claims decisions are contested by the people they affect. A record that states only the outcome is not a defence.
What is written down is the evidence set, the contradictions within it, and the confidence the assessment carried at the moment it was made.
